Update to Referral and Authorization Process

As of April 30, the Alternative Referral and Authorization form will no longer be available. Use the online referral management system on the provider portal, Availity. You can still submit your referrals and authorizations requests via fax to 866-852-1893. For newborn and NICU patients, please fax the face sheet to 866-852-1885.
