How to Report Fraud and Abuse

You’re an important partner in fighting fraud and abuse. If you suspect fraud or abuse, please take the time to report it.

When creating a report, ask yourself these questions:

Although not required, you’re also encouraged to include your name, phone number, and email address in the report.
